School Based Influenza Clinics For Attleboro Public Schools

The Attleboro Public Schools and the Attleboro Health Department are again partnering to offer school based influenza clinics this year. The permission form should be filled out completely and returned to the school nurse by October 13, 2011. Influenza vaccination is voluntary, not mandatory in Massachusetts. These clinics are only for students.

Forms can be printed from the Attleboro Public School website www.attleboropublicschools.com or obtained from the Health Department or the school Nurses. Please remember to fill out the form completely or your child will NOT be vaccinated. This year we will collect insurance information on the Vaccine Administration Record and bill insurance companies through Commonwealth Medicine. Your child will be imminized whether or not insurance information is provided.

Middle and High School students will be vaccinated during school hours.

There will be a flu clinic for elementary school students held on Thursday, October 27, 2011, from 3:30 p.m.-5:30 p.m. at Thacher Elementary School, 7 James Street, Attleboro. This clinic will be held on a first come, first served basis. Parents MUST accompany their child to this clinic. A make up clinic will be held at Attleboro City Hall one evening at a time to be announced for thos students ill or absent the day of the scheduled clinics.
